Need Help To add text variable in url

I need to have a form on a page where the content of a single text field is inserted into a URL

base url: http://www.domain.com/VARIABLE

The script needs to replace VARIABLE and output to URL like:

http://www.domain.com/data or
http://www.domain.com/anything

Any help appreciated.
