You could use this php code to add simple validation:

And add

<?php include 'password_protect_page.php'; ?>

at the top of index.php

Or you could use htaccess to password protect it.

Yes I see the nfo, and yes just CHMOD it to 755.

Nice script there
But you should work on making it more secure! Like it checks for extensions while uploading but not while viewing! So one can view the php file on the server
