An unmodded up-to-date phpBB3 is secure, there hasn't been any reports of a clean phpBB3 install being hacked.
Many webmasters make the mistake of installing or adding any modification they think is good, it's not just the number of modifications you have, it's the quality also.
A board can have one modification installed and get hacked, that if the modification wasn't secured, up-to-date and/or officially released.
I advise any webmaster using any type of forum script to only use officially released modifications, and stay up-to-date, whether it's with the forum script or the modification, that's the most important thing.
There are general precautions that you can take, like password protecting your moderator and administrator control panel (ie, with htaccess), but the most important thing is keeping it clean, and up-to-date.
Notice how many times I mentioned stay up-to-date, yes, it's that important.

As el_jentel1 said, keeping the installed mods and scripts is the most important aspect.
Other than that, phpbb3 is the bestThis is the staff, you have been banned

yes almighty el_jentel1 said it correct. I also would like you add some point 1. Check your cache folder regularly and pugre it from ACP ( as cache is 777 ) 2. Do not give your FTP access , on the hosting server to any person .. Eg: If i have access to FTP on your demo site .. which is http://somewarezsite.com/demo/ I can still hack into somewarezsite.com .. So just be careful
